Jason Jablonski

Preceptor, Administrative Director – Administration 

Jason has served as the Director of Pharmacy at SJHS since 2001. He was recently promoted to Administrative Director and now has oversight over Pharmacy, Performance Improvement, Infection Prevention, Respiratory Therapy, Neuro Diagnostics, Wound Care and the Sleep Disorder Centers. Prior to coming to SJHS he was the Pharmacy Director at IU-Health LaPorte for four years. During his tenure at SJHS, Jason and his staff have led many significant changes and improvements, including the implementation of an EMR, transition to a new hospital and the rollout of smart pumps and bedside barcode medication administration. He is the past Chairman of the Trinity Health Pharmacy Council, Chairman of the Pharmacy and Therapeutics Committee, Chairman of the Institutional Review Board and serves on numerous other local and Trinity-wide councils and work groups.
