Senior Services
We go above and beyond to improve the health of seniors in our community. Our Senior Services program offers extra care for area seniors, whether they are inside one of our hospitals, in recovery, homebound or focused on preventive care for a healthy, active life.
A Senior Services Navigator is available to help guide patients through the healthcare system. Together with our community health outreach initiatives, we can assist with:
- Education on a new diagnosis, disease or medical condition
- Finding a new physician or specialist
- Arranging assisted living or home care
- Financial help with medications
- Organizing medication delivery from a local pharmacy
- Exercise and nutritional information
- Referrals to support groups for chronic disease management

Advance Directives Assistance with arranging end-of-life decisions and living will documents with our Pastoral Care Department.
Billing & Account Assistance Personal hospital account assistance through our Patient Financial Services and Eligibility Assistance Department.
Diabetes Resource Center & Support Group Manage your diabetes and find support
Health Screenings Free tests and screenings provided by our Community Health and Well-Being team, including:
- Blood Pressure
- Body Fat
- Glucose
- Total Cholesterol
Lifeline with Auto Alert for Falls Provided through our Saint Joseph Health System VNA Home Care.
Mobile Medical Unit Clinic on wheels offers greater access to care in the community. Mammograms available onboard.
Oncology Nurse Navigator Patient navigator for cancer patients in partnership with Michiana Hematology Oncology.
Senior Fit Wellness and fitness for ages 55+, with physician approval.
Smoking Cessation Class Quit smoking for good with this 8-session class.
Telecare Daily telephone calls to the homebound through our Volunteer Services.
Volunteer Services Opportunities to volunteer in one of our SJHS facilities.
