Descriptions of funds for patient care and support

Fund Community Health Worker outreach services. Support health education activities for the community such as a food pantry, senior exercise classes, health fairs and other implementation strategies identified by the Community Health Needs Assessment

Help patients experiencing lung cancer get the compassion and understanding they need while navigating their treatment plan. Patients can have access to a special nurse navigator, who provides education, emotional support and guidance. This service is not covered under insurance programs.

Allow us to provide healthcare services "on the go" to communities in St. Joseph, Marshall and surrounding counties through our MMU, which reaches out to both urban and rural areas where it’s convenient for patients. Services include digital mammography, health screenings and corporate wellness programs.

Fund a special nurse navigator, who provides education, emotional support and guidance to help women who experience breast cancer and need compassion and understanding while navigating their treatment plan. This service is not covered under insurance programs.

Help parents with an infant in NICU who may live more than an hour away and/or be unable to afford the cost of a hotel room to stay close to their infant. Our goal is to improve overall health outcomes of premature babies and support parents in forming stronger bonds. Parents also have earlier access to professional and medical assistance, addressing infants’ developmental and medical needs.
