PACE Care Team

The PACE team will:

• Develop a specialized, individualized healthcare plan for and with each participant.
• Manage all healthcare services for participants.
• Help participants to live safely in the community.

These experts provide valuable skills and experience so program participants remain comfortably at home. They take the time to get to know each participant individually.

PACE team members

• Physician spends more time with their patients and treats them so they may stay independent and healthy at home.
• Nurses also spend more time with program participants than in other settings. Changes in health are watched closely.
• Social workers provide ongoing help and answers for seniors and their families.
• Activities staff offer many healthy and fun activities, including cognitive engagement.
• Registered dietician helps participants to eat healthy meals and learn to prepare at home.
• Personal care staff help with personal care needs at home and at the center.
• Physical and occupational therapists provide programs to achieve and maintain the highest levels of physical ability.
• Transportation staff is trained to meet the special needs of seniors.
• Chaplain supports the spiritual needs of participants and families as needed.

Services provided by Saint Joseph PACE

Services must be authorized by the care team, although emergency care does not need to be authorized. Our services include:

• Medical and specialty care, including a doctor who is an expert in senior care
• Nursing care (including at home when authorized)
• Drug coverage
• Transportation
• Help with meals and nutrition
• Occupational, physical and speech therapy and equipment, when authorized
• Support for caregivers and family
• Home and personal care
• Meals, exercise and fun activities
• Dental care
• Vision care
• Hearing care
• Podiatry
• Mental health services
• End-of-life care
• Services such as lab work, X-rays and surgery, when authorized
• 24-hour access to our team
