What is Saint Joseph PACE?

PACE is an acronym for Programs of All-inclusive Care for the Elderly. The goals of Saint Joseph PACE are to foster independence and health, preserve dignity and maintain individuals in their community as long as possible.

What is the PACE team?

The PACE team is a group of professionals who work with you to provide your healthcare services. The PACE team includes a primary care physician; nurses; physical, occupational and recreational therapists; social workers; home health aides; dietician; drivers and pastoral care counselor.

Where is Saint Joseph PACE located?

The majority of our services are provided at the center, or at home as needed. A typical day at the center may include nursing, primary healthcare and occupational and physical therapy. Hot lunches are provided. Transportation to the PACE Center is part of our services. Each van is equipped with electric lifts for wheel-chair-bound and physically disabled participants.

What if I have a problem when the center is closed?

Saint Joseph PACE staff are on call 24 hours a day, 7 days a week, including holidays. After hours, call 574.247.8700.

What if I need hospitalization or 24-hour skilled nursing care?

In the event of an emergency hospitalization, Saint Joseph PACE will assist the hospital with your coordination of care and manage your follow-up care. If you require rehabilitation and/or a short-term nursing home stay, the PACE team must authorize this care. You will continue to be enrolled in the PACE program and our staff will communicate with the rehab/short-term facility staff to coordinate your care and assist in your return to your home and to the PACE Center.

Who can join Saint Joseph PACE?

  • 55 years of age or older
  • In need of nursing-home level of care
  • Able to live safely in the home with the services of PACE

What does Saint Joseph PACE cost?

  • If you are on (or eligible for) Medicare and Medicaid, there is no cost to you.
  • If you are on traditional Medicare, but not Medicaid, you will have a monthly premium.
  • If you are on Medicaid only, there is no cost to you.
  • For participants who do not qualify for Medicare and/or Medicaid, there is a flat monthly fee.

Services provided by Saint Joseph PACE:

Services must be authorized by the care team, although emergency care does not need to be authorized.*Our services include:
