We strive to make patient registration as simple and convenient as possible. When a test or surgical procedure has been scheduled, you can pre-register by telephone for any of our facilities.

Be prepared to provide:

• Date of surgical procedure or test
• Demographic information
• Insurance information (on card)
• Complete physician name(s)
