MyChart FAQ

MyChart offers you personalized and secure online access to your medical records. It securely enables you to use the internet to help manage and receive information about your health. With MyChart, you can:

  • Schedule appointments
  • View your health record
  • View test results
  • Request prescription refills
  • Access trusted health information resources
  • Communicate with your care team
  • Pay your bills online
  • Confirm your appointment and electronically check-in to complete pre-visit paperwork online before your appointment
  • Get care through e-visits and video visits
  • Download the Trinity Health MyChart app to keep track of your health using your smart phone and/or tablet. Note: some features are not available when accessing your account via the app

MyChart provides you with a summary of your health information based on your doctor's entries in your patient portal. You can view your future and past appointments, test results, current health issues, medications you are taking, allergies, immunizations and medical history.

MyChart provides an easy way to ask your doctor basic, non-urgent questions using secure electronic messaging. This way you can receive an answer without repeated calls. Having a convenient summary of your health information, such as when you are due for preventive tests like a mammogram or prostate exam, helps you be a more active partner in maintaining and improving your health.

We encourage you to sign up for your MyChart account at your next doctor’s visit. At your visit you will receive a MyChart instant activation link, valid for 24 hours, via text or email. 
Another way to register is using an activation code found on your After-Visit Summary sheet or Billing Correspondence. Go to mychart.trinity-health.org and register using the activation code.

If you did not receive an activation code, lost it or it expired, you may call your healthcare provider to get one or ask to sign up during your next office visit.

Also, at any time, Trinity Health offers a self-service sign up for those who do not have an activation code. If the information entered matches the information we have in our system exactly as it appears, you will receive an email containing an access code. The MyChart Support Line, 574-203-5097, is available 7am - 7pm, 7 days a week to answer your MyChart questions and assist with access.

You will need a computer or mobile device that can connect to the Internet and an up-to-date browser, such as Google Chrome, Microsoft Edge, Safari or Firefox.

You must be 12 or older to use MyChart. Individuals 12-17 years of age must have parental or guardian consent or be an emancipated minor.

Yes, you may access some family members record, depending on your relationship. Proxy access allows a parent, guardian, or power of attorney to log in to their personal MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form through your health care provider to request access to this convenient service.

No. MyChart is a free service offered to our patients.

If your MyChart account is with a different health care system, you will need to create a new account specific to our health system. MyChart is utilized by many health care providers and systems throughout the country. Fortunately, if you have an active patient portal account at another organization that uses MyChart, you will be able to link your accounts.

Your name and email address will be treated with the same care and privacy given your health records and will never be sold or provided to anyone else. MyChart complies fully with federal and state laws related to privacy.

Yes! With MyChart Bill Pay, you’ll have access to make secure payments right through the MyChart application. You’ll receive a confirmation message in your MyChart message center. You do not have to have a MyChart account to pay your bill. You can pay as a guest from the MyChart page.

No, there is no charge to use our MyChart Bill Pay functionality.

If you currently have a patient portal with our health system, some of your historical health information will be transferred over a period of time to your MyChart account and be visible to you. Your provider will have access to your entire record. New information from visits after October 2021, including prescription refills, visit notes and lab results will be in your MyChart patient portal. You will still be able to access your historical medical information through your old patient portal, but no new information will be added. It will become read-only.
