We believe you should have simple access to your medical information. That’s why Saint Joseph Health System created My Health Records to access your records from Mishawaka and Plymouth Medical Center and My Care Connection to access your medical records from Saint Joseph Physician Network (SJPN) provider. With these patient portals, now you can access medical records and view test results from the convenience of your computer or mobile device. For any assistance with accessing My Health Records please call our support center at 877.448.1767. For any assistance with accessing My Care Connection please call our support center at 1.877.621.8014.

To obtain a paper copy of your medical records, click here to complete the form. All sections must be completed. Forms can be submitted by mail or by fax. Addresses and fax numbers are located at the top of the form.
