Saint Joseph Health System offers several online tools that make accessing your medical record and managing your healthcare easier.

The security of your personal information is our top priority. All medical records are protected by federal, state and hospital privacy laws. We believe you should have simple access to your medical information.

  • My Health Records - access your records from Mishawaka and Plymouth Medical Centers.

Enrollment questions

My Health Records and My Care Connection are patient portals that offer patients personalized and secure online access to portions of their medical records. It enables you to securely view information about your health. With My Health Records and My Care Connection, you can view:

  • Reason for hospitalization
  • Care team
  • Procedures performed during admission
  • Current and past problem list
  • Current medication list and medication history
  • Current medication allergy list and medication allergy history
  • Vital signs at discharge
  • Laboratory test results
  • Summary of care
  • Care plan
  • Discharge instructions

No. This is a free service offered to our patients.

Patients who wish to participate will be issued an invitation during their clinic or hospital visit. To create the invitation, we will ask you to provide your email address. A valid picture ID is required to verify your identity. This invitation will be emailed to you and will enable you to log in and create your own user name, password, and set-up your security questions.

For My Health Records, you may also self-register here.

An invitation will be sent to the email address you gave during sign-up. Once you have received the email, follow the directions on the screen. The step-by-step process will allow you to verify your identity by answering a challenge question and entering your date of birth. You will then be guided through the process to build your own user name and password.

MyHealthRecord has completed a security update. This update will make accessing your medical information online, through a computer, tablet or smartphone, safe and easy. It will also provide you with a better user experience and enhanced security. There have been no security breaches with your current account.

We invite you to re-register your account with MyHealthRecord and follow these instructions:

  • Verify your identity by entering your legal first name, legal last name and date of birth.
  • Verify your email address and set up a username and password.
  • Select three security questions. Be sure to supply answers that are not easy for others to guess or discover.
  • If your first name, last name and date of birth that was used for registration matches an existing patient record, a PIN will be sent to your phone that you'll need to enter online to verify your identity.
  • Once your identity is verified, you'll be able to access your patient record.

Questions about your medical record

Your test results will be viewable in the My Health Records or My Care Connection patient portals immediately after the results are put into your health record. The following test results are currently not available to view: tests indicating sexually transmitted diseases, HIV, hepatitis serologies, pregnancy test and tumor markers.

Tests of a very sensitive nature are not released through the My Health Records or My Care Connection patient portals.

Yes. Under Account Settings, select "change settings with Cerner Health" and select the Health Record tab to add your personal information. This information is for your own reference and will not be part of your hospital medical record. You may share this self-entered information by selecting "Share This Record."

Accessing medical records for your family

With MyHealthRecord, you can now access the hospital medical records of a loved one in order to help manage their care. My Care Connection does not yet offer the ability to see your loved ones records.

  • To access your medical records, both you and your proxy must create new accounts with MyHealthRecord and then establish your proxy relationship.

 

To create your new account you must:

  • Verify your identity by entering your legal first name, legal last name and date of birth. 
  • Verify your email address and set up a username and password.
  • Select three security questions. Be sure to supply answers that are not easy for others to guess or discover.
  • If your first name, last name and date of birth used for registration matches an existing patient record, a screen will prompt you to send for a PIN to your phone and this will let you enter it online to verify your identity. 
  • Once your identity is verified, you'll be able to access your patient record.


Once you have created your MyHealthRecord accounts, you can establish a proxy relationship by:

  • Obtaining the proxy's user ID.
  • Going into the proxy dashboard in your MyHealthRecord account.
  • Select the Grant Proxy Access button
  • Asking for proxy access and submitting the proxy's user ID. This will send a notification to the proxy and allowing them to accept this request.

You and your spouse will have separate accounts.

Technical Questions

Click the "Forgot password" link on the sign-in page to reset your password. You will be prompted to answer the security question that you set up when you signed up for your account. If you need further assistance, contact toll-free My Health Customer Support for technical support, including password resets.

You may also call 877-448-1767 (My Health Records) or 1-877-621-8014(My Care Connection) to request a new, secure password.

Go to the myHealth sign-in page. Log into your My Health account and go to your account settings to change your security questions. If you need further assistance, contact toll-free My Health Customer Support for technical support, including password resets.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure invitation process, personal IDs and passwords. Each person controls their password, and the account cannot be accessed without that password. Furthermore, My Health Records and My Care Connection use the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session.

My Health Records and My Care Connection are owned and operated by Saint Joseph Health System and Cerner Corporation. They are fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given your health records and will never be sold or leased by My Health Records or My Care Connection.

We aim to protect your privacy and security of your information. While logged in, if your keyboard remains idle for 20 minutes or more, you will be automatically logged out. We recommend that you log out of your session if you need to leave your computer for even a short period of time.

You need access to a computer connected to the Internet and an up-to-date browser (such as Safari, Chrome or Firefox).

Accessing your medical records with health management apps

Health Management mobile apps give you the ability to view some of your medical records (or someone else's records, where you have proxy rights) on a smartphone or tablet.
